SiCKO is more like a controlled howl of protest than a documentary. Toning down the rhetoric of past efforts--no CEOs, congressmen, or celebrities were accosted in the making of this film--Michael Moore's latest provocation is just as heartfelt, if not more heartbreaking. As he clarifies from the outset, his subject isn't the 45 million Americans without insurance, but those whose coverage has failed to meet their needs. He starts by speaking with patients who've been denied life-saving procedures, like chemotherapy, for the most spurious of reasons. Then he travels to Canada, England, and France to see if socialized medicine is as inefficient as U.S. politicians like to claim--especially those who receive funding from pharmaceutical companies. Moore finds quality care available to all, regardless as to income. He concludes with a stunt that made headlines when he assembles a group of 9/11 rescue workers suffering from a variety of afflictions. When Moore is informed that detainees at Guantánamo Bay--technically American soil--qualify for universal coverage, he and his companions travel to Cuba to get in on that action. It's a typically grandstanding move on Moore's part. And it proves remarkably effective when these altruistic individuals, who've either been denied treatment or forced to pay outrageous costs for their medication, experience a dramatically different system. It's impossible to walk away from John Q. without thinking about the film that could have been. The pathetic state of health care in the U.S. and the desperate behavior it engenders is not only worthy but edgy material; no doubt director Nick Cassavetes (She's So Lovely) and Denzel Washington (as well as Robert Duvall, Ray Liotta, James Woods, and Anne Heche) were drawn to the provocative pitch. The only snag is that John Q. has about as much edge as an after-school special. Washington plays John Quincy Archibald, a hard-working factory worker whose house stands to be repossessed and whose lovely wife (Kimberly Elise) is at her wits' end. When his extremely cute son collapses while rounding the bases in a Little League game, things go from bad to worse. John Q. takes a downtown Chicago emergency room hostage when he learns that the heart transplant his son needs won't be performed because his health care doesn't cover it. The action-drama that ensues--replete with one-liners, stilted debate, inept snipers, and multiple references to O.J. Simpson's white Bronco--is so littered with clichés that the issues, timely ones, get lost in a crescendo of melodrama. --Fionn Meade

Each expert paints a dismal picture of American birthing and emphasizes the frequent overuse of medical procedures in what are otherwise potentially normal deliveries. Stressing the prevalent use of midwives in birthing in other developed nations (70% of births are attended by midwives in Europe and Japan, versus 8% in the U.S.), the documentary then follows Cara Muhlhahn, a certified nurse midwife in New York City, as she attends a variety of home births. The footage is candid and sometimes very graphic, showing various home-delivery methods, including water birth. Interviews with Cara and her clients emphasize their shared philosophy on birthing as a normal life process that, when attended by a caring and well-trained midwife, can be both empowering and exhilarating. Though a midwife is often characterized as a supportive, but medically untrained birth attendee, the film dispels that stereotype, stressing a good midwife's solid training and knowledge of when it's appropriate to seek outside medical intervention. Key in every birth is a commitment to doing what's best for mother and baby, regardless of pre-planned agendas. The filmmaker's lament is that hospitals and doctors often too quickly advocate medical intervention in the interest of saving time and avoiding potential litigation. While unquestionably advocating midwifery over hospital birthing, this documentary presents solid expert opinions, concrete facts and statistics, and anecdotal experiences of both mothers and midwives that are crucial in making an informed decision about the use of midwifery in birthing as well as enlightening as to the current state of birthing in the United States. --Tami Horiuchi
